Understanding Home and Community-Based Caregiving
Home at Last of Brevard specializes in hands-on care and habilitative supports for people with special needs. We can help you increase independence in daily living, including self care, personal hygiene, community integration and recreation activities.
We have 5 group homes across Brevard County, offering personal support programs, companion services and respite care. We also offer Medicaid Waiver programs for those who prefer to get long-term care services in their home or community, rather than in an institutional setting.
Our goal is to help you become as self-sufficient as possible
Empower your loved one living with a developmental disability!
The Path to Community Integration and Quality of Life
Individual support can take many forms, from Medicaid Waiver supported services to home and community-based care. Contact us if you need support with an individual family member or friend with limited/restricted abilities that prevent them from handling daily activities.
Home at Last of Brevard is a veteran-owned and operated business. Our services are designed to create a sense of community, and to provide exceptional care.
We work with each patient to create the best care plan for their needs
